Trilogy Sports & Fitness collects health history, profile information, emergency contact details, waivers, intake forms, and other client information to help provide safe, organized, and appropriate training, coaching, fitness, sports performance, and related services.
This Health History and Profile Information Intake Policy explains what information may be collected, how it may be used, and the responsibility of clients, parents, and guardians to provide accurate and updated information.
By creating an account, submitting an intake form, completing a health history form, signing a waiver, purchasing a service, booking a session, registering for a program, or participating in services through Trilogy Sports & Fitness, you agree to this policy.
For most profile updates, health history updates, injury updates, emergency contact changes, account questions, form issues, billing concerns, membership issues, scheduling issues, or general support needs, clients should submit a support ticket through our Support Request page at https://trilogyfitpt.com/support. This allows Trilogy Sports & Fitness to properly track, review, and respond to requests.
1. Purpose of Health History and Profile Information

Requirement to Complete Intake Forms
Clients may be required to complete health history forms, profile information forms, waivers, releases, consent forms, emergency contact forms, and other required intake documents before participating in services.
Participation may be delayed, denied, paused, or canceled if required forms are incomplete, inaccurate, outdated, expired, or not submitted.
Completing intake forms does not guarantee that participation is risk-free or that Trilogy Sports & Fitness can safely accommodate every injury, medical condition, limitation, or special request.
4. Accuracy of Information
Clients, parents, and guardians are responsible for providing complete, accurate, and truthful information.
This includes information related to:
Health history Injuries Pain Medical conditions Limitations or restrictions Emergency contacts Age or grade level Account information Contact information Billing information Training goals and experience Any changes that may affect safe participation
Trilogy Sports & Fitness relies on the information provided by clients, parents, and guardians. If information is inaccurate, incomplete, outdated, or withheld, Trilogy Sports & Fitness may not be able to properly assess risk, adjust training, communicate effectively, or respond in an emergency.
5. Responsibility to Update Information
Clients, parents, and guardians are responsible for updating Trilogy Sports & Fitness if health, contact, emergency, account, or profile information changes.
Updates should be submitted as soon as possible through the Support Request page at:

This includes updates related to:

Injuries, Pain, Illness, or Medical Concerns
Clients and athletes should not participate in training if they are injured, ill, medically restricted, or experiencing pain that may make participation unsafe.
If a client or athlete has an injury, illness, medical restriction, or pain concern, the client, parent, or guardian should notify Trilogy Sports & Fitness before participating by submitting a support ticket through https://trilogyfitpt.com/support.
Clients and athletes should also immediately notify a trainer or staff member if pain, dizziness, shortness of breath, discomfort, injury, illness, or any unusual symptoms occur during training.
Trilogy Sports & Fitness may pause, modify, or deny participation if we believe training may be unsafe or inappropriate.
7. Medical Disclaimer
Trilogy Sports & Fitness is not a medical provider. Our services are not medical advice, medical treatment, diagnosis, physical therapy, rehabilitation, or a substitute for care from a licensed healthcare provider.
Information collected through health history forms, intake forms, support tickets, assessments, or staff conversations is used to help support safer training decisions and business operations. It should not be considered medical evaluation or medical clearance.
Clients, parents, and guardians should consult a physician or qualified healthcare provider before participating in training if there is any medical condition, injury, limitation, illness, pain, or concern that may affect safe participation.
8. Medical Clearance
Trilogy Sports & Fitness may request medical clearance, doctor approval, return-to-play documentation, or other written documentation before allowing participation or continued participation.
Medical clearance may be requested when a client or athlete has:
A current or recent injury Pain or recurring symptoms Surgery history Concussion history Significant illness Heart, breathing, neurological, orthopedic, or other medical concerns Doctor restrictions Return-to-play restrictions Any condition that may affect safe participation
Failure to provide requested medical clearance may result in delayed, modified, paused, or denied participation.
9. Emergency Situations
Clients, parents, and guardians are responsible for providing accurate emergency contact information.
In the event of an emergency, Trilogy Sports & Fitness may contact emergency contacts, call emergency services, provide relevant information to emergency responders, and take reasonable action to protect the safety of the client or athlete.
Trilogy Sports & Fitness is not responsible for delays or issues caused by inaccurate, outdated, or missing emergency contact information.
10. Minor Participants
For minor athletes, a parent or legal guardian is responsible for completing health history forms, profile information, waivers, consent forms, emergency contact forms, and any other required intake documents.
By submitting information for a minor, the parent or guardian confirms that they have the authority to provide the information and consent to participation.
Parents and guardians are responsible for updating Trilogy Sports & Fitness about any injury, illness, medical condition, restriction, emergency contact change, or other important update involving the minor athlete.
11. Use of Intake Information
Trilogy Sports & Fitness may use health history and profile information for:
